The continuous presence of the Greek e-journal of Perioperative Medicine in the scientific literature, over last 21 years, has been gradually increased and this is attributed to your help and we would like to thank you again for this continuous support.
The Greek e-journal of Perioperative Medicine in its last issue for this year covers a wide area of interests.
The review article of Pantelidis A et al. decribes the three fundamental mechanisms that contribute to the development of acute sepsis-related kidney damage (S-AKI). It emphasizes also on the rapid diagnosis of S-AKI with the use of new biomarkers with enhanced sensitivity in renal function assessment. The authors note that although the prevention of S-AKI is often impossible, early and specific use of antibiotics together with the treatment of sepsis could reduce the risk of developing S-AKI.
The article of Boskou I et al. attempts to gather the evidence presented in the literature that dictate the use of the Transversus Abdominis Plane block technique as an effective method for postoperative pain relief after laparoscopic and robotic assisted laparoscopic total hysterectomy.
Next the experimental study of Grosomanidis V et al. investigates and records the effects of increased intraabdominal pressure (IAP) alone and in combination with sepsis on respiratory mechanics in pigs. The authors concluded that IAP increase had detrimental effects on respiratory system mechanics, which were reversible after IAP normalization. They also noted that the coexistence of sepsis caused further adverse effects, which remained even after IAP normalization.
The following case report of Tzima M et al. presents the perioperative management of a parturient in the 3rd trimester of pregnancy with a sizeable adrenal mass, which was successfully and uneventfully resected.
The final article of Papadopoulou D et al. presents the anaesthetic management of a super-obese patient who underwent Open Reduction Internal Fixation (ORIF) for proximal humeral fracture. The authors focused on the technical difficulties that often arise in obese individuals, concerning both the performance of peripheral nerve blocks and the airway management. They also emphasised the need of systematic management plan consistent with guidelines in order to to minimize the failure risk in these patients.
