The editorial board of the Greek e-journal of Perioperative Medicine would like to emphasize that our goal is ensure free-access, high quality, published medical articles.

Our first issue of the Greek e-journal of Perioperative Medicine for 2026 covers a variety of articles concerning perioperative medicine.

The editorial by our Assistant Editor Aslanidis Th focuses on Point-of-care ultrasound (POCUS), a rapidly expanding bedside diagnostic tool that enables quick clinical decision-making, particularly in emergency and critical care settings. The author notes that its widespread adoption has outpaced regulatory frameworks, leading to variability in training, certification, and legal policies—especially in countries like Greece. Although no legal cases have been linked to performing POCUS, failure to use it when indicated has been implicated. A growing number of protocols exist, varying in complexity and application. Finally, in order to ensure safe and effective use, the author emphasizes on the need for standardized governance, training, and quality assurance systems.

Intensive care unit of “Agios Pavlos” General Ηospital of Thessaloniki, started its function in December of 2002. The present paper is a brief presentation of the activity of the unit till February 2026 (23 years). The second issue of the Greek e-journal of Perioperative Medicine for 2025 presents articles concerning perioperative medicine.

The review article of Aidoni Z. et al describes modern fluid management strategies in the treatment of shock. The authors emphasize on individualized and goal-directed therapy versus liberal fluid administration. They note also that hemodynamic monitoring, markers of fluid responsiveness are key components of current practice, aiming to restore tissue perfusion and oxygen delivery while minimizing complications.

Shock represents an acute, life-threatening condition characterized by inadequate tissue perfusion and oxygenation, ultimately leading to multi-organ failure. Timely and targeted restoration of intravascular volume through fluid resuscitation remains a cornerstone of initial management. However, the traditional strategy of aggressive fluid administration has come under scrutiny in recent years, due to the risks associated with fluid overload and its related complications. Ten to twenty percent of the patients hospitalized in an Intensive Care Unit (ICU) will require tracheostomy, either due to long term hospitalization or due to other medical conditions. The procedure itself, as well as the afterward care; carry a lot of potential complications for the patients, some of which may be preventable, if certain precautions are being taken. The decision for the type of the procedure, percutaneous tracheostomy or surgical tracheostomy along with the appropriate tracheostomy tube type is based on the individual characteristics of the patient, ie anatomical characteristics, clinical stability and expertise available.
