The uninterrupted and continuous presence of the Greek e-journal of Perioperative Medicine in the scientific literature is attributed to your help. On behalf of the editorial board, we would like to emphasize that our goal is ensure free-access, high quality, published articles in the area of medical press and we would like to thank you again for this continuous support. The participation of all of us with high quality publications will enable our journal to continue to be part of international libraries.

The second issue of the Greek e-journal of Perioperative Medicine for 2025 presents articles concerning perioperative medicine.

The review article of Aidoni Z. et al describes modern fluid management strategies in the treatment of shock. The authors emphasize on individualized and goal-directed therapy versus liberal fluid administration. They note also that hemodynamic monitoring, markers of fluid responsiveness are key components of current practice, aiming to restore tissue perfusion and oxygen delivery while minimizing complications.

The next article of Al Kagiet S. et al focuses on identifying and categorizing the key criteria, that must be met before decannulate an ICU patient, which is considered crucial for developing effective and safe decannulation protocols. The authors emphasize on the… Continue reading

Shock represents an acute, life-threatening condition characterized by inadequate tissue perfusion and oxygenation, ultimately leading to multi-organ failure. Timely and targeted restoration of intravascular volume through fluid resuscitation remains a cornerstone of initial management. However, the traditional strategy of aggressive fluid administration has come under scrutiny in recent years, due to the risks associated with fluid overload and its related complications. Ten to twenty percent of the patients hospitalized in an Intensive Care Unit (ICU) will require tracheostomy, either due to long term hospitalization or due to other medical conditions. The procedure itself, as well as the afterward care; carry a lot of potential complications for the patients, some of which may be preventable, if certain precautions are being taken. The decision for the type of the procedure, percutaneous tracheostomy or surgical tracheostomy along with the appropriate tracheostomy tube type is based on the individual characteristics of the patient, ie anatomical characteristics, clinical stability and expertise available. Airway management and ventilation are considered cornerstones for the survival of critically ill patient both in the pre- and in the in-hospital setting. Endotracheal intubation [ΕΤΙ] is the gold standard technique for definitive airway management. ETI application in the prehospital setting is associated with incidents and complications which can be attributed on objective difficulties but also on the education and training level of prehospital staff members. Tracheal tumors are rare neoplasms that can present with symptoms mimicking more common respiratory conditions, such as asthma. We report a case of a 71-year-old male who presented to the emergency department with severe respiratory distress initially diagnosed as status asthmaticus. Despite aggressive conventional asthma therapy, the patient remained in respiratory failure requiring intubation and mechanical ventilation. Continue reading
